Fall Registration packets are available at Lamppost Pizza located on Warner, east of Brookhurst. Follow instructions enclosed in envelope.

Follow this link to www.eayso.org follow the instructions on how to fill out the form online. You must then print out the form, sign it and mail it to Region 5.

Fill in division according to the age/division chart

Put siblings or cousin requests in appropriate box on form.

Fee for all player for the fall season is $85.00 per player for the first two player from the same family. The rest of the players from that family are free.

If you are new to Region 5, include a COPY of proof on date of birth. (birth certificate, passport, etc.)

Make cheacks payable to AYSO Region 5.
