Early Registration Discounted Fees:
U5 to U19:		$85 per player
Full Registration Fees (after May 13, 2006):
U5 to U19:		$100 per player

All players signing up must have:

1. Name, address & phone number of emergency contact

2. Parent/guardian signature required on sign-up form

3. Birth certificate required unless returning Region 5 player

4. Check or cash
