REGION 5 | Misconduct Form

» MISCONDUCT INFORMATION:

All Misconduct Reports for players and coaches/ spectators should be filed to Miles Nomura, RRA by the end of the day the game occurred. Prompt reporting is necessary as some teams may play the following Day, thus the referees need to be promptly notified of any ineligible players and/ or coaches. Also, prompt reporting is the only effective way we can properly enforce game suspensions and identify inappropriate behavior. If it's not reported, it didn't happen, thus the problem is handed off to the next referee and the perpetrator will think his behavior was permissible.

FOR ANY SEND-OFFS please call Miles Nomura as soon as possible the same day at (714) 898-0213 with the following information:

(1) Game info (date/time/field/division);
(2) Team identification (region/coaches name);
(3) Player identification (jersey number/name/reason for send-off), and/or
(4) Coaches identification (name/team/reason for send-off)

Please complete the Report and attach it to an e-mail to Miles Nomura with a subject line "Misconduct Report".
